Camp Registration Form

Thank you for booking a Print and Play camp! We're so excited to have your child join us.

Before your child's first day of camp, we require a completed registration and waiver form for each child attending. This form includes important medical information and the liability waiver required for participation.

How to Get Your Registration Form

A PDF registration and waiver form will be emailed to you within 24 hours of your booking. Jaime will send it personally to the email address you used at checkout.

If you'd like to get a head start, or haven't received the form within 24 hours, please email us at: makeit@printandplay.ca

Please use the subject line: Camp Registration -- [Child's Name]

What the Form Includes

Please have the following ready when completing the form:

• Child's full name and date of birth
• Emergency contact information
• Medical information (allergies, medications, any conditions we should be aware of)
• Signed liability waiver (parent or guardian signature required)

What to Expect Next

Once we receive your completed registration form, Jaime will be in touch closer to your camp start date with everything you need — including what to bring, drop-off and pick-up times, and any other important details.
